Pursuant to the Guarantees Acknowledgement Act of Alberta, all personal indemnities must be accompanied by a signed Certificate of Notary Public. (Exception: sole proprietors signing on behalf of themselves are exempt from this requirement.)

- Copy of the Will, if applicable
- Copy of application to Court or Office of the Public Guardian and Trustee
- Affidavits and/or Court Orders
- Copies of all related correspondence

- Copy of the Management Plan, approved by the Public Guardian or Trustee / Scheme of Management of Estate Assets
- Copy of the Application to Replace the Public Guardian and Trustee as Statutory Guardian of Property
- Documentation from the Public Guardian and Trustee Office with respect to the appointment or refusal of the applicant as Statutory Guardian
- Copy of Will, if applicable
- Consent(s) to appointment of Guardian by next of kin/Consents of beneficiaries

- Copy of letter from Transfer Agent confirming the stop transfer, must be dated within last twelve (12) months
- Copy of the duly executed in triplicate and notarized Affidavit of Loss
- If this submission involves an estate matter, certified notarial copies of all relative documentation granting legal fiduciary representation; ie - Letters Probate, Letters of Administration, Granted Powers of Attorney, Trusteeship etc. must be provided
- If applicant is a public corporation, copy of latest year-end financial statements
- If applicant is a private company, copy of latest year-end financial statements and Personal Net Worth Statement of all major shareholder(s)
- If applicant is an individual, Personal Net Worth Statement
- Certificate of Independent Legal Advice may be required
Waiver of probate bonds
Up to $100,000

Please submit the following documents with your application when additional signature is required on the Indemnity Agreement
- Personal Indemnitors Signature Page
- Corporate Indemnitors Signature Page
- Notarial copy of deceased’s Death Certificate and Copy of the Will, if applicable
- Copy of letter from Transfer Agent confirming the requirement for Waiver of Probate Bond (Letter of Indemnity); must be dated within last twelve (12) months
